Request for Payment form (2 forms per page).

This form is required to be completed before a payment can be made, unless the payment  is a regular payment that has had automatic pre-approval by the Board . Members responsible for such payments will be informed of this.

Note: All payments require pre-approval by the Board. Please do not spend money and ask for reimbursement, unless you have Board approval to do so. Board approval is a requirement that is rigorously checked by the Auditor.
